Caso Clínico: Paciente masculino de 63 años con antecedente de glaucoma en ambos ojos, en tratamiento tópico con brimonidina desde hace 15 meses, concurre al servicio de oftalmología por ojo rojo, sensación de cuerpo extraño y fotofobia de 10 días de evolución. Al examen físico su agudeza visual (AV) era de 8/10 en ojo derecho (OD) y 10/10 en ojo izquierdo (OI) mejor corregida. A la biomicroscopia presentaba edema e hiperemia en borde palpebral, intensa hiperemia conjuntival, reacción papilar en conjuntiva tarsal inferior, reacción en cámara anterior positiva, y precipitados retroqueráticos granulomatosos en ambos ojos. La presión intraocular medida con tonómetro de Goldman resultó en 14 mmHg para ojo derecho (OD) y 15 mmHg para ojo izquierdo (OI). El examen de fondo de ojos (FO) resultó sin particularidades en ambos ojos. Se solicitó una serie de estudios complementarios para descartar otras causas de uveítis, resultando todos dentro de parámetros normales. Se planteó el diagnóstico de uveítis anterior granulomatosa bilateral secundaria a brimonidina tópica y se indica reemplazo de colirio antiglaucomatoso y tratamiento con esteroides tópicos suaves, con respuesta favorable y resolución completa del cuadro. Conclusión: En el caso presentado, la uveítis anterior granulomatosa bilateral se resolvió por completo tras la suspensión de la brimonidina y la administración de tratamiento tópico antiinflamatorio, confirmando la relación causal con el fármaco. Caso clínico: Paciente femenina de 65 años consultó por crecimiento acelerado de una lesión sólida preexistente en ojo derecho. Presentaba una lesión pigmentada en conjuntiva que invadía el área corneal de 7,5 mm. Tras realizar una evaluación mediante ultrabiomicroscopía, se procedió a su biopsia escisional (técnica no-touch) y cuatro ciclos de tratamiento posoperatorio con mitomicina C al 0,04% cada 6 horas durante 7 días con una semana de intervalo de descanso. El diagnóstico histopatológico e inmunohistoquímico confirmó un melanocitoma epitelioide pigmentado (condición extremadamente infrecuente en la conjuntiva ocular) con pocos casos reportados previamente en la literatura. A los 5 meses de seguimiento oftalmológico y oncológico, la paciente no tuvo recidivas, su visión se encuentra conservada y se mantiene un seguimiento cercano. Conclusión: Se describe un caso poco frecuente de melanocitoma epitelioide pigmentado conjuntival que pudo ser tratado y controlado de manera local. Serán necesarios más casos para considerar la utilidad de coadyuvantes como la mitomicina C tópica. Sin embargo, incluso con DALK se producen reacciones inmunitarias epiteliales, subepiteliales y estromales que pueden ocasionar un rechazo inmunológico. En un reciente estudio de Kasamatsu y colaboradores —donde analizaron 464 ojos con DALK— detectaron signos de rechazo en 19 ojos cuya manifestación principal fue el edema estromal en el 100% de los casos, además de hiperemia ciliar en el 89,5%, precipitados queráticos (68,4%), edema epitelial (68,4%), infiltrados (47,4%), opacidad corneal (21,1%) y desprendimiento de la membrana de Descemet (10,5%)2. Su aparición se asoció a desencadenantes clínicos específicos como la reducción/cesación de esteroides tópicos o la retirada de suturas. En la imagen se observa el aspecto de la córnea de un paciente masculino de 28 años con antecedente de DALK en ojo izquierdo a los 11 años, remitido por su contactóloga por intolerancia al uso de lentes de contacto. En la biomicroscopía se destacan infiltrados subepiteliales, opacidad corneal y vascularización centrípeta estromal en tres cuadrantes como signos sugestivos del rechazo. También se presentan imágenes de la tomografía de coherencia óptica del segmento anterior, observándose la interfase profunda entre el estroma residual del huésped y el tejido injertado. Con un tratamiento intensivo y continuo con prednisolona tópica se logró controlar el cuadro, que permanece estable con seis meses de evolución. Si bien el rechazo del DALK puede presentarse en cualquier momento de la vida, su detección y tratamiento oportuno pueden ayudar para preservar y extender la viabilidad del injerto por más tiempo conservando la función visual del paciente.
