Se lo sometió a trabeculectomía y como resultado tuvo una amplia mejoría de la agudeza visual a través de su remanente central. Al realizar una revisión del tema, nos encontramos que es frecuente la mejoría de la agudeza visual, del campo visual y de la capa de fibras nerviosas debido a la intervención de factores como la posición de la porción anterior del nervio óptico, la elasticidad de la lámina cribosa y su composición, factor determinado por la edad y el papel de la posición de la vasculatura como determinante del flujo de irrigación de los axones del nervio óptico. Sin embargo, esto no conviene utilizarse como objetivo en la reducción de la presión intraocular ya que la importancia radica en la no progresión del daño. Métodos: Se presenta el caso clínico de una paciente con síndrome de Down y desprendimiento seroso macular secundario a foseta papilar. Se realizó una retinopexia neumática con gas C3F8 seguida de fotocoagulación láser en el borde del disco óptico una semana después. Se evaluó la agudeza visual y el grosor macular mediante tomografía de coherencia óptica durante el seguimiento. Resultados: La paciente experimentó una mejoría significativa de la agudeza visual y una reducción del desprendimiento macular a los 2 meses de la intervención. Estos resultados se mantuvieron estables durante los 3 años de seguimiento. No se observaron complicaciones asociadas al tratamiento. Conclusión: La retinopexia neumática con fotocoagulación láser diferida puede ser una alternativa eficaz y segura en el tratamiento del desprendimiento seroso macular asociado a foseta papilar, especialmente en pacientes con contraindicaciones o limitaciones para cirugías más invasivas. Se trata de una paciente de 11 años que consultó en el Hospital de Pediatría Garrahan (Buenos Aires, Argentina) por presentar episodios repetidos de sangrado ocular derecho que aparecían y cedían de manera espontánea. La paciente fue internada para valoración interdisciplinaria. Durante la internación se realizaron valoraciones clínicas de múltiples especialidades y estudios complementarios solicitados por cada una de ellas sin obtener ningún resultado anormal. Tiempo después de su alta y con la ayuda del servicio de salud mental se pudo arribar al diagnóstico de causa psicógena postraumática asociado a situaciones de abuso y bulling escolar. Si bien la hemolacria psicógena es poco conocida y muy poco descrita en la literatura, conocer esta entidad rara nos permite formular diagnóstico en aquellos pacientes donde la valoración clínica es normal pero los episodios de sangrado no mejoran. A su vez, las lágrimas de sangre pueden representar un signo clínico alarmante que advierte sobre situaciones que pueden poner en riesgo la vida del paciente y que pueden ser tratables, como en el caso presentado. Title A name given to the resource Trasplante lamelar anterior profundo (DALK) : manifestación clínica del rechazo estromal Creator An entity primarily responsible for making the resource Calvo Braun, Elisa Constanza Description An account of the resource El trasplante lamelar anterior profundo (DALK) es una interesante opción quirúrgica que elimina la posibilidad de rechazo endotelial del aloinjerto, que es la principal causa de fracaso del injerto tras la queratoplastia penetrante1. Sin embargo, incluso con DALK se producen reacciones inmunitarias epiteliales, subepiteliales y estromales que pueden ocasionar un rechazo inmunológico. En un reciente estudio de Kasamatsu y colaboradores —donde analizaron 464 ojos con DALK— detectaron signos de rechazo en 19 ojos cuya manifestación principal fue el edema estromal en el 100% de los casos, además de hiperemia ciliar en el 89,5%, precipitados queráticos (68,4%), edema epitelial (68,4%), infiltrados (47,4%), opacidad corneal (21,1%) y desprendimiento de la membrana de Descemet (10,5%)2. Su aparición se asoció a desencadenantes clínicos específicos como la reducción/cesación de esteroides tópicos o la retirada de suturas. En la imagen se observa el aspecto de la córnea de un paciente masculino de 28 años con antecedente de DALK en ojo izquierdo a los 11 años, remitido por su contactóloga por intolerancia al uso de lentes de contacto. En la biomicroscopía se destacan infiltrados subepiteliales, opacidad corneal y vascularización centrípeta estromal en tres cuadrantes como signos sugestivos del rechazo. También se presentan imágenes de la tomografía de coherencia óptica del segmento anterior, observándose la interfase profunda entre el estroma residual del huésped y el tejido injertado. Con un tratamiento intensivo y continuo con prednisolona tópica se logró controlar el cuadro, que permanece estable con seis meses de evolución. Si bien el rechazo del DALK puede presentarse en cualquier momento de la vida, su detección y tratamiento oportuno pueden ayudar para preservar y extender la viabilidad del injerto por más tiempo conservando la función visual del paciente.
