Si bien se trata de una rara presentación del trauma ocular contuso, es la causa más frecuente de este fenómeno. En reportes previos, la conducta más frecuente ha sido la extracción inmediata del cristalino con el reparo escleral y de lesiones en segmento posterior coexistentes. Sin embargo, en nuestro caso, durante la exploración quirúrgica no se evidenció una masa lentigiforme circunscrita, observándose únicamente restos de material fibrinoide inespecífico asociado a tejido cicatricial sobre una esclera aparentemente adelgazada. Por la evolución de nuestra paciente y los buenos resultados obtenidos, sugerimos que, ante la presencia de un facocele y la ausencia de comorbilidad retiniana, diferir el procedimiento quirúrgico primario debido a la probabilidad de tener una herida autosellada y realizar una cirugía más segura en un ojo con menor inflamación.
