Sin embargo, incluso con DALK se producen reacciones inmunitarias epiteliales, subepiteliales y estromales que pueden ocasionar un rechazo inmunológico. En un reciente estudio de Kasamatsu y colaboradores —donde analizaron 464 ojos con DALK— detectaron signos de rechazo en 19 ojos cuya manifestación principal fue el edema estromal en el 100% de los casos, además de hiperemia ciliar en el 89,5%, precipitados queráticos (68,4%), edema epitelial (68,4%), infiltrados (47,4%), opacidad corneal (21,1%) y desprendimiento de la membrana de Descemet (10,5%)2. Su aparición se asoció a desencadenantes clínicos específicos como la reducción/cesación de esteroides tópicos o la retirada de suturas. En la imagen se observa el aspecto de la córnea de un paciente masculino de 28 años con antecedente de DALK en ojo izquierdo a los 11 años, remitido por su contactóloga por intolerancia al uso de lentes de contacto. En la biomicroscopía se destacan infiltrados subepiteliales, opacidad corneal y vascularización centrípeta estromal en tres cuadrantes como signos sugestivos del rechazo. También se presentan imágenes de la tomografía de coherencia óptica del segmento anterior, observándose la interfase profunda entre el estroma residual del huésped y el tejido injertado. Con un tratamiento intensivo y continuo con prednisolona tópica se logró controlar el cuadro, que permanece estable con seis meses de evolución. Si bien el rechazo del DALK puede presentarse en cualquier momento de la vida, su detección y tratamiento oportuno pueden ayudar para preservar y extender la viabilidad del injerto por más tiempo conservando la función visual del paciente.
