Caso clínico: Se presenta un paciente masculino de 17 años de espectro autista con disminución progresiva de la agudeza visual y un déficit alimentario que determinó un cuadro de neuropatía óptica bilateral secundario a un déficit de cobalamina. El diagnóstico se llevó a cabo a través de un exhaustivo interrogatorio que detectó un déficit alimentario acompañado de estudios complementarios oculares y sistémicos. Tras determinar una anemia macrocítica y niveles de vitamina B12 menores a 50 pg/ml, se inició un tratamiento con suplementación intramuscular de complejo vitamínico B de forma semanal por 4 semanas. Luego se continuó vía oral, logrando la detención del deterioro de la agudeza visual del paciente sin mejoría de ésta. Conclusión: El diagnóstico diferencial de neuropatía óptica nutricional debe ser investigado mediante un exhaustivo interrogatorio, considerando los antecedentes y los hábitos del paciente, además de la correcta exploración oftalmológica. Tras el diagnóstico, es un cuadro que, tratado oportunamente, puede controlarse pero no siempre revertirse, como ocurrió en este caso.
