Si bien tiene una presentación y manifestaciones similares, se debe diferenciar de cuadros como el síndrome de Stevens-Johnson, la necrólisis epidérmica tóxica o el eritema multiforme, ya que el tratamiento y la evolución son diferentes. Nuestro objetivo es presentar el caso de un paciente masculino con esta patología. Caso clínico: Se presenta a la guardia un varón de 21 años con compromiso sistémico mucocutáneo y síntomas respiratorios que fue derivado a oftalmología por dolor ocular bilateral. En la anamnesis refirió fiebre, odinofagia, lesiones orales y oculares, disnea y hemoptisis. El examen físico reveló úlceras extensas en mucosa oral y conjuntivitis con escleritis anterior. Estudios por imágenes mostraron infiltrados pulmonares compatibles con proceso infeccioso y se descartaron múltiples etiologías infecciosas comunes. Ante la sospecha de mucositis y rash inducidos por micoplasma (MIRM, por su sigla en inglés), se solicitaron anticuerpos específicos con resultado altamente positivo para Mycoplasma pneumoniae. Se instauró tratamiento con ceftriaxona, claritromicina y metilprednisolona intravenosa además de terapia tópica ocular. El paciente evolucionó favorablemente sin secuelas respiratorias, orales ni oculares al control de 7 días y fue dado de alta con seguimiento ambulatorio. Conclusión: Este caso ejemplifica la manifestación clínica del MIRM, señalando las claves para establecer el diagnóstico oportuno y una intervención terapéutica adecuada mediante un abordaje multidisciplinario.
