Presentar un caso clínico de quemadura ocular severa con abordaje quirúrgico integral precoz, destacando su evolución y correlato con la literatura actual. Caso clínico. Varón de 33 años con quemadura ocular mixta por agua y líquido refrigerante hirviendo, con opacidad corneal total, simbléfaron severo e isquemia limbar inferior de 180°. Se realizó abordaje quirúrgico integral en un único tiempo con desbridamiento conjuntival, colocación de membrana amniótica y lente de Illig. La evolución mostró revascularización limbar progresiva, adecuada epitelización, preservación anatómica y recuperación visual, alcanzando AV sin corrección de 7/10 a los 21 días. Conclusión. En este paciente, la intervención quirúrgica temprana con desbridamiento conjuntival, membrana amniótica y lente de Illig permitió controlar la inflamación aguda, prevenir el simbléfaron y recuperar visión útil en tres semanas, preservando la integridad de la superficie ocular.
