Describir un caso de queratitis neurotrófica con perforación corneal secundaria al abuso de anestésicos tópicos, resaltando la importancia del diagnóstico precoz y la educación del paciente. Caso clínico. Paciente femenina de 34 años consulta por dolor ocular y fotofobia en el ojo derecho. Había estado utilizando proparacaína tópica cada 40 minutos, junto con antibióticos y lubricantes. Presentaba agudeza visual OD: cuenta dedos a 3 metros; OI: 7/10. En la evaluación se detectó perforación corneal. Se inició tratamiento antibiótico intensivo, atropina y se realizó recubrimiento conjuntival y parche tectónico. Conclusión. El uso no supervisado de anestésicos tópicos puede causar toxicidad epitelial severa, ulceración progresiva y perforación corneal, como sucedió en nuestro caso. La detección precoz y el abordaje terapéutico oportuno son fundamentales para preservar la integridad ocular.
